You or a loved one can be admitted from HOME to Lee Health & Rehab Center if you:

• You can be admitted from home within 30-days from discharge date from the hospital to Lee Health & Rehab
• If you and your doctor agree you would benefit from skilled care services, i.e., Physical Therapy, Occupational Therapy, Speech – Language Pathology, IV Therapy, and more…
• Some Managed Care Insurance does not require a 3-day Hospital Stay – (Original Insurance Cards required to verify Insurance Coverage)
