WOUND SPECIALIST: Kristi Hornsby, LPH : Certified Wound Care Nurse specializes in assessments, staging, treatment plans and monitoring pressure ulcers, venous stasis ulcers, diabetic ulcers, and surgical wounds. Kristi also works with the patient’s physician to assess the appropriateness of antibiotic therapy, wound VAC, compression wraps, as well as wound cleaning process that removes dead tissue and contaminants.
THERAPY SERVICES: Accelerated Care Plus “Modality-Based”; E-Stim, Ultrasound, Diathermy, Omni-Cycle; Physical Therapy/Occupational Therapy/Speech/Language Pathology, Vestibular Therapy
NURSING RATIO TO PATIENT:
LPN = 1/30 Short-Term Rehab = 1/20
CNA = 1/10 7:00 pm 1/15
RN = 2/110 Week Days 1/110 Weekends/Nights
TOTAL OCCUPANCY = 110 SNF BEDS
20 SNF/ICF Short-Term Rehab Wing
30 SNF/ICF Secure Memory Unit
60 SNF/ICF Beds
NURSING SERVICES: C-PAP, BI-PAP, Telemetry monitoring, foley catheter, suprapubic catheter, urostomy, surgical drains, nebulizer treatments, suctioning-portable, tracheostomy management – 4-5 days post-op, CPR, AED
ENTERAL THERAPY: G/J tube feedings/care; NG tube feedings/care-SHORT TERM; rectal tube;
IV THERAPY: PICC management/central lines, IV insertion, IV antibiotics, IV push medications (furosemide, etc), CAD pumps, TPN
DIAGNOSTIC TESTING: Mobile Imaging-EKG studies , Lab work-STAT, Bladder ultrasound; Cardiac ECHO; Venous duplex; Sleep apnea – Physician has access to report(s) within 1-2 hours
DIALYSIS: Hemodialysis – Norton/Kingsport; Peritoneal Dialysis
CONSULTATIONS: Dental, Psychologist, Podiatry
INSURANCE: Humana, Secure Horizons, United Healthcare, United Mine Workers of America, Anthem BCBS, Tri-Care for Life, Champ-VA, other private insurances, Medicaid

REQUIRED DOCUMENTS PRIOR TO ADMISSION FROM HOSPITAL: Inform your hospital case manager you wish to transfer to Lee Health & Rehab Center. Hospital Demographic Sheet, History & Physical, Chest X-Ray, Medication List, Labs/Cultures, Infectious Disease Report, PT/OT/ST therapy notes; Hospitals in State of Virginia required to complete a UAI Screening (Uniform Assessment Instrument).
REQUIRED DOCUMENTS PRIOR TO ADMISSION FROM HOME: Copy of Insurance Cards, History & Physical from primary physician, Chest-X-Ray (within 6-mos), Medication List, Labs/Cultures, Home Health Agency clinical notes, UAI Screening.
