AHCA recognized Lee Health & Rehab earning the Tier one level of recognition for achieving and safely reducing hospital readmissions. 

Lee Health & Rehab has strived to minimize hospital transfers with indepth workshops focusing on recognizing, accessing and following through with physician interventions to prevent return to hospital.  Currently area hospitals are actively working with skilled nursing facilities that are able to demonstrate low hospital readmission rates. AHCA Quality Initiative is an effort that builds upon existing work of the long term care by setting specific, measurable targets to further improve quality of care in America’s skilled nursing centers and assisted living communities. The Quality Initiative embraces CMS’s Triple Aim of Better Health, Better Quality and Reduced Costs.
